The Vine Medical Centre are looking to recruit to the post of Care Coordinator to work within the Vine Medical Centre/ Primary Care Network multidisciplinary healthcare team.
You will be part of a team that coordinates the successful achievement of all contractual requirements and support collaborative working between multiple organisations, to achieve improved outcomes for the Practice / PCN population.
The Care Coordinator role is responsible for the co-ordination and delivery of specific projects to ensure the achievement of key health and welfare objectives of the surgery.
You will work alongside other clinicians in the practice and as part of a multidisciplinary team. They will take responsibility for an area of work that will contribute to fulfilling the requirements of the Network Contract Directed Enhanced Service (PCN DES).
You will provide a point of contact for patients and clinicians in coordinating care and will help to deal with incoming queries relating to medication from patients or other healthcare providers.
The position is a full time post of 37.7 hours per week and salary will be depending on experience.
