This course is open to all Cancer Care Co-ordinators, Social Prescribers, Care Navigators based in Primary Care, within Kent & Medway, who support patients living with a cancer diagnosis.

Understanding Holistic Needs Assessments and how to implement them into Primary Care at the three months contact point (QOF CAN005)
Cancer treatments and understanding what happens in Secondary Care
